Our mission is to improve access to mental health services and ensure exceptional patient outcomes through innovative technology and compassionate care.Job Summary: We are seeking a dedicated and empathetic Mental Health Care Coordinator to join our team. The Care Coordinator will play a crucial role in ensuring our patients receive comprehensive and coordinated mental health care. This position involves interacting with patients, mental health providers, and other team members to facilitate seamless healthcare delivery and enhance patient satisfaction.Key Responsibilities
Patient Communication:
Serve as the primary point of contact for mental health patients, addressing inquiries to get into care, providing information on model of care, and ensuring a positive experience.
Care Coordination:
Review patient needs and match with the right therapist best suited to treat their needs and schedule the intake session.
Personally connect to the client to ensure comfort with Joon processes and intake attendance.
After the intake, coordinate scheduling ongoing sessions through the platform and address scheduling challenges that may arise during treatment.
Monitor patient progress and ensure adherence to treatment plans, providing support and resources as needed.
Administrative Duties:
Maintain accurate and up-to-date patient records, ensuring confidentiality and compliance with healthcare regulations.
Handle administrative tasks such as data entry, documentation, and reporting.
Support billing team with collecting payments and submitting claims as needed.
Problem Solving:
Address and resolve any issues or concerns related to care, working proactively to prevent potential problems.
Escalate complex situations to clinical leadership when necessary
Patient Advocacy:
Advocate for patients’ needs and preferences, ensuring they receive personalized and compassionate care.
Provide education and resources to patients and their families to help them understand their mental health conditions and treatment options.
Qualifications:
Bachelor’s degree in psychology, social work, healthcare administration, or a related field preferred.
Minimum of 2 years of experience in a healthcare or telehealth setting, with a focus on patient coordination and support.
Strong interpersonal and communication skills, with the ability to build rapport with patients and mental health providers.
Excellent organizational and multitasking abilities, with attention to detail.
Proficiency in using electronic health records (EHR) and telehealth platforms.
